I will collect personal information about you only as is necessary for your treatment. This may include your name, contact details, emergency contact details and referral information, as well financial and insurance information for billing purposes. I will also record information about your treatment which is a special category of personal data under the law, meaning that it must be handled even more sensitively.
The confidentiality of your medical information is important to me, and I make every effort to prevent unauthorised access to and use of your information. I must comply with the data protection legislation and relevant guidance when handling your personal information. Your personal data must be managed in accordance with the law and the principles of this privacy notice.
I will not share information about your treatment with others without your consent, except when I am concerned about your safety, and in most cases I will inform you before I do this.
I am the data controller for the information I hold about you. You have the right to access this information and to correct any errors. Please contact me if you need to do this. I will keep your information for at least seven years.
